Thank you for scheduling an appointment with TeleEMC for patient __________ on _______ at ______

Upon the patients arrival please have the iPad charged, then click on the message icon to notify us of the impending TeleVisit (I.e. “patient Smith has arrived and shall be starting TeleEMC shortly”).

If the patient is either late (more than one hour), or a “no show” for their scheduled appointment, and should they need to be rescheduled this requires us to be notified via text or email message, so as to change the appointment time in the EMR. Failure to do so will result in a kiosk error upon patient registration.

On the iPad please click on ECW Kiosk icon and log in with the staff user name and password credentials provided to you, then hand the IPad to the patient to complete the necessary identification, review of systems, injury questions, and required documents to be executed. When completed the iPad shall be returned to the front desk by the patient, and TeleEMC is ready to begin.

Please escort the patient to an exam room where chrome book is set up.

Press Gmail icon on the shelf of the desktop and open the email with the subject: “Telemed Appointment Confirmation” please make sure it says hello to the proper patient and then click the orange “join this Telemed appointment” button, at this time a form for vitals will be displayed please attempt to collect as much of this information as possible then press “submit”. Then the patient is placed in a virtual waiting room, and a TeleEMC provider will be online, and on the screen momentarily.

Although it is not required, we ask that the chiropractors office fax us a copy of their initial exam chart, and any imaging reports for us to attach to the file, for a more comprehensive EMC determination (TeleEMC fax # 866-324-4956).

The executed EMC determination letter will be sent to you via email or fax within 24 hours of the examination.

Chiropractors are not required to agree to the terms and conditions
or receive hardware to receive TeleEMC services.
