Schedule on our website by scrolling down to the page footer and clicking “Homevisit”.
Please make sure to fill out all required fields accurately on the form and click next, then click submit.
Print or save a copy of the appointment confirmation page.
Same day appointments are always available, and welcome.
Step 2) Patient registration upon attendance:
This step must be completed before initiating the TeleVisit.
We offer two ways to complete this step:
Option A) Online form
Option B) Download form
While on teleemc.com, go to the top of the webpage and click “patient forms” in the header menu.
Then click “online forms”. Please ensure all pages are signed accordingly.

After clicking “patient forms” in the header menu, click “download forms”.
Once filled out, either fax them to:
(866) 324-4956, or scan them by using the “upload forms” link, which can also be found under “patient forms”.
The documents must be sent to TeleEMC before going to step 3.
Please ensure all pages are signed accordingly.
Step 3) Connecting the TeleEMC provider to the TeleVisit via email:
You will receive an e-mail 30 minutes prior to the scheduled appointment time.
Click the red “CONNECT WITH PROVIDER” button - if the red button fails to appear, your email service is blocking images. Please unblock to proceed or use the in-text link.
A screen requesting your vital signs will appear. While collecting this information is beneficial to an EMC, it is not required and can be skipped by clicking the “next” button in the bottom left corner of the screen.
Click “Start TeleVisit”.
You will be placed in a virtual waiting room with a notice on the screen showing the provider’s name.
When the provider connects, you may be asked for access to the Microphone and Camera. Click “Allow”.
The provider will connect live and proceed with the examination.
If the provider does not come on screen within 5 to 10 minutes, please call 866-466-4362 and select option 2. We will insure a prompt response.
